Welcome to Resource Health Services

The Home and Community Based (HCS) Program is an individualized alternative to traditional facility or institutional based programs that care for large numbers of people. This is a Medicaid Waiver program that assists persons with developmental Disabilities to live life as normally as possible in a home setting.

Eligibility Requirements
  1. Must have the diagnosis of Mental Retardation or Related Condition.
  2. Must have a current level of care and level of need.
  3. Must have a current individual plan of care.
  4. Must be currently receiving SSI with Medicaid.
  5. Must have HCS slot offered to individual if not currently enrolled into HCS services.
